Our Services


Our combined knowledge and skills of our interdisciplinary team prides itself on treating patients like family and providing care to help you or your loved return to everyday life.  We work towards our patients regaining their independence with more than 21 different programs, including those in education and hands-on disease management training.
 


Programs include:

Diabetes – Designed to help patients and their families understand the disease process by educating them on how to keep their diabetes under control. Nurses visit the patient and teach them the benefits of diet, exercise and medication to manage their diabetes.

Congestive Heart Failure – Patients who have been diagnosed with congestive heart failure (CHF) will learn ways to help take control of their disease. Qualified nurses visit to discuss the importance of diet and exercise, share additional education on newly prescribed medications and answer all questions.

Wound Care – Pressure wounds, complex surgical wounds and ulcers are just a few of the conditions addressed in this program. Patients are taught appropriate positioning, pain management and how to improve their functional ability. Our skilled nurses will help with additional education and training for families and caregivers on the patient’s treatment plan.

IV Therapy – This program was developed to help patients return home quickly from their hospital stay. We partner our patients with IV-nurses, who have the skills to administer IV therapy in the comfort of the patient’s home. Amicita nurses are always available to answer questions and make sure the patient, as well as family members, are comfortable with the IV and important procedures for handling IV’s.

Orthopedic’s  – This program was designed to help treat patients who have sustained an orthopedic injury or disability – such as orthopedic surgery, fracture, joint replacement or degenerative condition (i.e. arthritis). Our program focuses on helping patients improve movement and strength and reaching their highest level of functional ability.

Telehealth  – Telehealth home monitoring program (currently only offered by the Albany & Arlington office) is an advanced home monitoring service for patients diagnosed with medical conditions that require close observation. The program enables healthcare providers to daily monitor patients with the goal of increasing patient awareness and self care. Telehealth will help to reduce unnecessary doctor’s visits, emergency room visits and hospitalizations through promoting guidance, education and evaluations. Amicita uses state-of-the art equipment designed by Honeywell HomMed© a leader in the telehealth and telecommications industries.
